What Is Personal Support Services?

Primecare provides Personal Support Services (PSS) for individuals with physical or functional disabilities or who are medically fragile. Our caregivers assist with daily living activities such as bathing, dressing, meal preparation, and mobility, ensuring comfort, safety, and independence in the home environment.

Who Qualifies for GAPP and How to Get Started

GAPP supports children under 21 with medical complexities who need skilled nursing or personal care at home. Our team helps guide families through the application process to make accessing care simple and stress-free.

Who Is Eligible?

How to Apply

  1. Complete a GAPP Connect Card – This form collects your child’s basic information.
  2. We assign a coordinator based on your location and your child’s needs.
  3. We submit your application to the state for processing—typically within 30 days.
  4. Once approved, your child may receive up to 40 hours of weekly care.

Your Skilled Nursing Questions—Answered

Families receive full support from our coordinators, nurses, and caregivers. We educate parents, provide updates, and ensure services fit the child’s medical and emotional needs. Our goal is to keep children safe, supported, and thriving at home.
Children with conditions like cerebral palsy, epilepsy, tracheostomy/ventilator dependence, cystic fibrosis, muscular disorders, and Type 1 diabetes may qualify for skilled nursing support.
PSS caregivers assist with bathing, feeding, dressing, toileting, mobility, and safety. This service is for children who need day-to-day assistance but may not require constant medical care.
For Skilled Nursing (GAPP), PrimeCare helps with paperwork, assessments, and Medicaid approval, typically within 30 days. For PSS, eligibility is based on need and Medicaid guidelines, and our coordinators walk families through the process step by step.
