Who Is Eligible
- Under 21 years of age
- Have straight Medicaid
- Have a physical or functional disability or medical complexity
How to Apply
- Complete a GAPP Connect Card with your child’s basic information.
- A coordinator is assigned based on your location and your child’s needs.
- We submit your application to the state (typical review ~30 days).
- Once approved, your child may receive up to 40 hours of weekly care.

- Bathing, grooming & dressing
- Toileting & continence care
