Who Qualifies for GAPP and How to Get Started
Who Is Eligible?
- Are under 21 years of age
- Have straight Medicaid
- Have a physical or functional disability, or are medically fragile
How to Apply
- Complete a GAPP Connect Card – This form collects your child’s basic information.
- We assign a coordinator based on your location and your child’s needs.
- We submit your application to the state for processing—typically within 30 days.
- Once approved, your child may receive up to 40 hours of weekly care.
